Intensive Mental Health Services

 

The Center for Aging Resources (CFAR) announces intensive mental health services with older adults in parts of Los Angeles County and all of San Diego County. In partnership with the Los Angeles County Department of Mental Health, these extended services are delivered through a Full Service Partnership for Older Adults Program. In collaboration with the San Diego County Department of Mental Health, these personalized, flexible services are provided by way of a Full Service Partnership for Older Adults, Mobile Senior Outreach Teams, and Traditional and Institutional Case Management Programs. The Center will now expand and extend its history of field-based mental health service to seniors by providing “whatever it takes” to support older adults with the most serious mental health difficulties.

Who can we serve:

Elders whose optimal independence has been jeopardized by at least one of the following:

· Homelessness and imminent risk of homelessness

· Hospitalization and risk of acute psychiatric hospitalizations

· Incarceration and risk of going to jail

· Imminent risk of placement at a Skilled Nursing Facility or nursing home

· Presence of co-occurring disorder

· Recurrent, serious risk of abuse or self-neglect

· Serious risk of suicide

Mental health services to be provided:

· Outreach and engagement

· Intake

· Assessment, including neuropsychological evaluation

· Care management

· Individual rehabilitation

· Medication evaluation and follow-up

· Crisis management

· Peer advocacy and support

· Client and caregiver support groups
